More Details About the Medicine

Lodopin 5 mg Tablet is a prescription medicine containing amlodipine, a calcium channel blocker used primarily to treat high blood pressure (hypertension). It is also used in the management of certain forms of angina (chest pain caused by reduced blood flow to the heart).

In Pakistan, Lodopin 5 mg is marketed by Martin Dow Marker Limited. Current Pakistani government procurement records identify Lodopin 5 mg as amlodipine besylate 5 mg, and pharmacy listings show it in packs of 20 tablets.

What Is Lodopin 5 mg Used For?

Lodopin is most commonly prescribed to control high blood pressure. It may be used alone or alongside other antihypertensive medicines when one medicine is not sufficient.

Its main uses include:

  • Hypertension (high blood pressure)
  • Chronic stable angina
  • Vasospastic (Prinzmetal) angina
  • Long-term cardiovascular management in selected patients with coronary artery disease

Pakistani product information consistently identifies hypertension and angina as its principal uses.

How Does Lodopin Work?

Amlodipine belongs to a group of medicines called dihydropyridine calcium channel blockers.

Calcium plays an important role in the contraction of blood-vessel muscles. Amlodipine reduces calcium entry into vascular smooth-muscle cells, allowing the arteries to relax and widen.

This lowers resistance to blood flow and consequently reduces blood pressure.

In people with angina, reducing the workload on the heart and improving coronary blood flow can help decrease episodes of chest pain.

Lodopin for High Blood Pressure

High blood pressure often causes no obvious symptoms, but persistently elevated blood pressure increases the long-term risk of problems such as:

  • Stroke
  • Heart disease
  • Heart attack
  • Kidney disease
  • Damage to blood vessels

Lodopin lowers blood pressure while it is being taken regularly. Feeling well does not mean hypertension has disappeared, so treatment should not be stopped simply because there are no symptoms.

Lodopin for Angina

Lodopin can also be prescribed to help prevent angina attacks.

Angina commonly causes pressure, tightness or discomfort in the chest when the heart muscle does not receive sufficient oxygen-rich blood.

Amlodipine can reduce the frequency of angina episodes, but it is not intended as an immediate-relief medicine for a sudden attack of chest pain.

New, severe or persistent chest pain requires appropriate medical assessment.

How to Take Lodopin 5 mg

Lodopin is generally taken once daily, but the exact dose should be determined by the prescribing doctor.

It can usually be taken:

  • With or without food
  • With a glass of water
  • At approximately the same time each day

Amlodipine has a long duration of action, which allows once-daily administration.

Do not take extra tablets because your blood pressure is high on a single reading unless your doctor has specifically instructed you to do so.

Usual Adult Dose

For hypertension or angina, 5 mg once daily is a commonly used adult starting dose of amlodipine.

Depending on blood-pressure control, response and tolerability, a doctor may adjust the dose. 10 mg once daily is generally the maximum adult dose.

Some patients may require a lower starting dose or more cautious dose adjustment.

The correct dose depends on the individual, so these figures should not be used to change treatment without medical advice.

How Quickly Does Lodopin Work?

Amlodipine begins affecting blood pressure after a dose, but its full blood-pressure-lowering effect develops gradually.

It is therefore a long-term control medicine, rather than something that should be taken only when a blood-pressure reading happens to be high.

Doctors usually assess blood-pressure readings over time before deciding whether a dose needs adjustment.

Common Side Effects

Many people tolerate amlodipine well. Possible side effects include:

  • Ankle or foot swelling
  • Headache
  • Dizziness
  • Flushing
  • Tiredness
  • Sleepiness
  • Palpitations
  • Nausea
  • Abdominal discomfort

Peripheral edema—swelling around the ankles or lower legs—is one of the best-known adverse effects of amlodipine.

Why Can Lodopin Cause Ankle Swelling?

Amlodipine dilates small arteries. In some people, this changes pressure within tiny blood vessels and allows fluid to accumulate in surrounding tissues, particularly around the ankles and feet.

This does not automatically mean that the heart or kidneys are failing.

However, significant or persistent swelling should be reported to the prescribing doctor. The doctor may review the dose, other medicines and possible alternative causes.

Do not start a diuretic or stop Lodopin yourself simply because ankle swelling develops.

Can Lodopin Make You Dizzy?

Yes. Dizziness or lightheadedness can occur, particularly after starting treatment or following a dose increase.

Getting up slowly from a sitting or lying position may help.

If dizziness is severe, causes fainting or is associated with very low blood-pressure readings, contact your healthcare professional.

Precautions

Tell your doctor before taking Lodopin if you have:

  • Very low blood pressure
  • Significant heart disease
  • Heart failure
  • Severe aortic stenosis
  • Liver disease
  • Previous allergy to amlodipine or related medicines

People with significant liver impairment may require more cautious dosing because amlodipine is extensively metabolized by the liver.

Drug Interactions

Amlodipine can interact with other medicines.

Tell your doctor or pharmacist if you take:

  • Other blood-pressure medicines
  • Certain medicines that affect amlodipine metabolism
  • Some antifungal medicines
  • Certain antibiotics
  • Some HIV medicines
  • Simvastatin
  • Medicines that can further lower blood pressure

Using several antihypertensive medicines together is common and may be intentional. However, the combination should be supervised to avoid excessive lowering of blood pressure.

Lodopin vs Lodopin-V

These are different products.

Lodopin contains amlodipine alone.

Lodopin-V combines amlodipine + valsartan, two blood-pressure medicines with different mechanisms of action.

For example, Lodopin-V 5/160 mg contains amlodipine 5 mg plus valsartan 160 mg.

A patient prescribed Lodopin-V should therefore not replace it with ordinary Lodopin assuming the products are equivalent.
